In 1962 the British scientist Winter found "moist therapy". In the experiment, Winter proved that when the woundsurface is in a moist environment, migration velocity of epithelial cells is significantly fast, which is faster than the dry environment . Because of this, the "moist therapy" high-tech products provide a moist healing environment through the use of a variety of different materials for wound. These products can not only absorb wound exudate to get adequate drainage, but also can retain exudate in whole or in part in the cover,resulting in a moist environment for the wound in local. Due to wet substrates have a positive effect on cell proliferation and the rate of epithelial cells. Epithelial cells can move rapidly from health skin to wounds to accelerate the wound healing.

The principle of sealing and thermal insulation

1, Prevent microbial into the wound;

2, Promote the wound moist, warm local environment;

3, Promote the synthesis of a variety of enzymes, activate the growth factor, accelerate healing;

4, Improve local microcirculation to "wake up" chronic wound cleaning ability;

5, Activate various types of cells (such as neutrophil phagocytosis of pathogenic microorganisms) in the treatment stage in moist environmennt;

6, Cut carbon dioxide concentration, increase angiogenesis and granulation formation;

The structure and function of the skin,

The structure of normal skin:

The skin from the outside to the inside can be divided into three layers, the table cortex (Epidermis), the dermis and subcutaneous tissue (Hypodermis).

The physiological functions of the skin:

1, the protection and defense function:

Provide mechanical protection;

The skin surface of the acidic environment (PH4.2~5.6) resistants to the action of microorganisms;

The chemical substances invasion resistants to a certain extent.

2, metabolism:

Through the sweat glands, sebaceous gland secretion and absorption function,  the skin together with the sugar,water, protein, lipid and many kinds of material takes part in metabolism to maintain the balance of fluid, electrolyte and protein;

3, body temperature regulation function:

Relying on its rich vascular network skin and sweat glands, participate in the regulation of body temperature in order to maintain a constant body temperature;

4, respiratory function:

The skin can discharge the carbonate, oxygen absorption, accounting for about 1%of lung metabolism under normal circumstances, which can account for the lung metabolic in high temperature or strong physical labor at the rate of 1.5~20%;

5, sensory function:

Nerve endings is rich in the skin, which can perceive pain, touching, heat, cold and so on.

Clinical analysis and characteristics of chronic wound:

The clinical wound healing is a complex and ordered biological process. Usually divided into wound formation,inflammation, wound reconstruction, epithelialization of the wound, wound remodeling five stages. (Figure)

Three, factor of wound healing:

1, Not appropriate local treatment: lack of basic knowledge of wound and improperly use various dressings;

2, Wound humidity: A variety of cells, enzymes and growth factors on wound can not occur in dry conditions. When in a moist environment, its activity has been strengthened to promote wound healing;

3, Wound temperature: The activity of various cells and enzymes on the wound at normal body temperature is the highest. The research shows that if the temperature is lower than the body temperature of 2 ¡æ, the activity of cells and enzymes reduce significantly;

4, Lack of blood supply on local wound: Increasing local wound pressure, friction, shear stress and local small vessel thrombosis formation make the wound blood supply decline, thereby affecting the wound healing;

5, Wound infection: wound infection can cause local tissue necrosis, impeding wound healing;

6, The formation of callus: Callus will affect the normal metabolism and migration of cells;

7, Wound pain: The pain will make blood vessels constriction around the wound, reducing tissue oxygen,affecting the reconstruction of vascular tissue.
